DiSC Cymru:
Digital transformation
in social care

Where we came from

DiSC Cymru began as a bold idea rooted in a simple but powerful belief: that digital could play a vital role in transforming social care in Wales.

In the early stages, the Chief Digital Officer for Local Government and the Chief Digital Officer for Health and Social Care jointly recognised the need for a unified approach to support digital transformation in the sector. This wasn’t just about technology – it was about people, services, and better lives.

It was clear that we were not starting from scratch, there was already a lot going on in this space in Wales, and beyond, but organisations were finding it difficult to navigate.

The DiSC Cymru framework was born out of a desire to bring the different pieces of the jigsaw together, understand where existing work could be shared and scaled, and collaboratively identify gaps. It aimed to bring together councils, care organisations, and national partners under one umbrella, to make digital work better for social care.

DiSC Cymru is built around four strategic pillars

These pillars guide everything from understanding services and users, building digital capabilities and fostering innovation, to influencing policy and ensuring social care has a strong collective voice at a national level.

The framework is evolving into a central hub for knowledge sharing, joint working, and coordinated delivery. It supports care organisations in using data, digital tools, and technology to improve outcomes for individuals receiving care, their families, unpaid carers, and the professionals who support them.

Where we’re going

Where we’re going

The journey ahead is ambitious and exciting. DiSC Cymru is not just a framework – it’s a movement. The next phase focuses on translating the partnership commitment to the framework into real action and delivering our programmes of work under each pillar.

The focus remains clear: to create a culture of continuous improvement, empowering care organisations, and delivering digital solutions that make a real difference.

We’re proud of the strong partnership we have developed, but to really make a difference, we need more input from those on the front line of social care delivery. We’d love to hear from you – what do you think of the framework, what’s good, what’s bad and what’s missing? Please share your thoughts here.
